    试用期终于过了。。“输液反应”

    今天考试考得好烂哦,本来就对那些基本知识不会过份的上心。今天就丢脸了,为此我特意搜索出来,以后都会记得了。。
     
    不过有空还是要多看书哦大笑
       
         1.发热反应
    1.1 发病特点 在输液过程中患者突然畏寒,不自主颤抖,迅速转为高热,严重发绀,面色苍白,重者心率快,脉细速,虚脱,多数经处理后迅速好转。发热反应是输液反应中最常见的一种,较常见的原因有致热原病菌(普通菌和毒菌等)或其他物质污染液体或输液用品,也可能因药物不纯或药物有配伍禁忌所致。
      1.2 防治和护理 在输液过程中,寒战反应一旦出现,应立即暂停或完全停止输液,并详细检查发生反应的原因。寒战发冷时保暖,立即注射非那根(25~50 mg)或其他抗过敏药物,艾灸百会穴等。发生虚脱时可针刺人中,严重时可注射肾上腺素(0.5~1 mg)。留观病人待退热至38℃以下,并无其他不适才可离开。

      2 血栓性静脉炎
      由于长期输注高渗葡萄糖液体和血管内膜药物所引起病变的静脉内膜发炎并导致静脉管腔内血液凝成血栓。
      2.1 主要症状 局部沿静脉径路上有红肿、触痛、热痛等,但一般无全身症状或仅有不适感。
      2.2 防治和护理 安慰病人消除紧张情绪,为病人更换注射部位使患肢休息,局部热敷,严重时局部用95%酒精或50%硫酸镁行热湿敷。

      3 急性肺水肿
      3.1 发病特点 输液过量或过快,特别是输入含钠液体过多时,容易发生急性肺水肿,原有心脏病,心功能不全者,肺功能不全者,老年体弱儿童输液应特别注意,发病时患者突然感到呼吸困难、气促、剧烈咳嗽、烦躁不安、口唇发绀,严重时口鼻可喷涌出大量粉红色泡沫样液,听诊两肺出现干湿性啰音,心音弱速。
      3.2 防治和护理 发现这种情况,应立即停止输液,取端坐位,两腿下垂,以减少静脉回流,减轻心脏负担,立即加压吸氧。湿化瓶给予20%~30%酒精,湿化吸氧,必要时静脉滴注氨茶碱,利尿药液以利尿排水。西地兰0.4 mg加50%葡萄糖40 ml缓慢静注等对症处理。

      4 过敏反应
      4.1 主要为药物性过敏性反应为多见 目前,在临床上多见于清开灵注射液、双黄连注射液、氧氟沙星类注射液,静脉输液时10 min~1 h左右病人出现皮肤荨麻疹、有瘙痒,重者胸闷、发热、口唇发绀,此时应注意休克发生。
      4.2 防治和护理 立即更换药液和输液管,即用5%葡萄糖维持输液,以便抢救用药,立即注射非那根25 mg,地塞米松5 mg,严重者给予吸氧,肌注肾上腺素1 mg,有过敏性休克者按抗休克处理。严密观察病人全身情况,直至症状完全消失才可离院。

      5  空气栓塞
      5.1 发病特点 由于输液管输液时空气未排尽,输液管连接不紧密,加压输液,连续输液接瓶时不及时而又没有注意重排空气,使空气进入静脉,随血流经右心房到右心室内阻塞肺动脉口,使血液不能进入肺内,引起严重缺氧。此时,病人感到胸闷,呼吸困难或严重发绀,听诊心前区可闻及一个响亮的“水泡声”。
      5.2 防治和护理 立即停止输液,通知医生,积极配合抢救,安慰病人,以减轻恐惧感。立即为病人至左侧卧位和头低足高位,以减少空气进入肺动脉口。并给予氧气吸入,严密观察病人全身情况,必要时对症处理。

      6 输液反应的临床调查
      调查了新洲社康中心2003~2005年的输液人数和在临床上发生的输液反应类型及结果,见表1。表1 2003~2005年输液人数和在临床上发生的输液反应类型及结果注:2003~2005年输液人数16880人,输液反应44人,占2.6‰
      以上调查资料说明一般的输液反应只要及时发现,及时处理,预后是良好的,极少有并发症发生。但是,输液反应也有可能会导致严重后果。所以,我们应强调预防为主的原则,护理人员应具备良好的职业道德,对病人的用药认真查对,对每一项无菌技术操作一丝不苟、精益求精。在病人输液过程中做到经常巡视,随时注意观察病人,如有可疑反应症状出现应立即暂停或更换液体,严密观察并及时采取必要的相应措施,才能确保病人的安全

    2007年提前总结

    好久好久没上传照片,更加久没在这发发牢骚哦,因为2007年快要结束了,我却在这一年里发生很多的事情,最重要的是做了一个以前跟自己说决不做的决定~~~
     
    新环境,新工作,用了将近三个月的时间才算是有点适应,但并不能说得上合适。。
     
    虽然到现在还是未能完全明白当时怎么会做出这样的决定,但路就是这样走开了,怎么艰辛还是要走下去的,努力吧,往好的方面想,总会走出一条合适的路的。。
     
    在这么痛苦的时期,真的好幸运地有人一直在支持,关心,开导,陪我走过来,让我在摇摆不定的思想斗争中,挺过来了。。
     
    还好最安慰与最享受的就是,辛苦的夜班之后,可以回到家,很简单的跟家人吃吃饭,聊聊天,还可以跟好朋友行街,食饭,无厘头地大笑聊天,可以说这就是辛苦上班的最大动力了,你会发觉辛苦上班,其实最后要求的东西会是最简单的,就是珍惜你的家人与朋友,这也是最好的减压方法。。
     
    人,好像总是贪心的,总会想着自己事业,爱情,家庭都顺利的,当然我也不例外,但往往事与愿违。。
    虽然事业才刚刚起了个头,但前面的路真的是蛮朦胧的,急也没用,你不慢慢走下去,还真不知道下一步该如何走去好。
    爱情,很顺其自然地展开了,看着好朋友的结婚照片,多多少少会些羡慕,不知自己的爱情树什么时候才会开花?不知道自己以后会不会也是一样的幸福?
    但也会非常的担心,毕竟两个人生活矛盾就会多起来,再加上两个家庭的牵涉,就会有莫名的恐惧。此时还真不知道是期待还是抗拒了。虽然这也是迟早的环节。。
     
    新一年快到了,期待着下一年更多的精彩及如意,平常心对待生活,在平凡的生活中活出不平凡吧,FIGHTING,Linda~~~~~

      Mr.Rathbone: Well, how are you doing1?

      Eugene: Fine, sir. The computer section is coming along2 nicely. I should be finished with it by this afternoon.

      Mr.Rathbone: Oh, that is great, great. But, I need to pull you off3 the computers for a little while and put you on something else4.

      Eugene: Something else?

      Mr.Rathbone: Yeah, yeah. Listen, you know about electricity, don't you?

      Eugene: Well, as that is the power source for most computers, I do possess a rudimentary5 knowledge of it of course.

      Mr.Rathbone: Oh, that is great, great! That's just what I wanted to hear.

      Eugene: Uh, why?

      Mr.Rathbone: Because of6 this.

      Eugene: The main circuit7 box?

      Mr.Rathbone: That's right. Notice something odd8 about it?

      Eugene: Well yes. None of the wires appear to be attached9.

      Mr.Rathbone: Nobody gets to the heart of a problem10 as quick as you. Up to now, we've been powering the place with a generator11, but it's time for that to stop12. I want you to fix the circuit box.

      Eugene: Fix it?

      Mr.Rathbone: Yeah, you know, attach the wires.

      Eugene: Uh, am I to understand that you wish me to wire your building?

      Mr.Rathbone: No, no. I don't wish you to do it. I want you to do it. You got everything you need here: transformer, wire, fuses13.

      Eugene: Mr.Rathbone, while I do know something about electrical current14, I am not a certified15 electrician.

      Mr.Rathbone: Did I say you had to be?

      Eugene: You don't understand. This job requires16 the services of a true professional17.

      Reggie: You bet4! It would be a first…I can see you becoming famous, Mr.Bumper.

      Reggie: We could write stories about school, our teacher, even about you!

      Bumper: It does sound terrific5, doesn't it?

      Reggie: It'll be a fantastic educational comic book, Mr.Bumper.

      Bumper: I like it! I like it! We'll do it, little Reggie! You'll produce a comic book about our wonderful school! Get some paper, pencil, Indian ink, brushes and pens. You've got work to do! Reggie, you could sell ice cubes6 to Eskimos7! I hope the first issue is as good as you say!

      C: (Miss Mood rushessintosthe office.)

      Miss Mood: What happened? Little Reggie's taken over8 the whole art room!

      Bumper: That's right, Miss Mood! We're going to do an educational comic book! Our school is about to go down in history!

      Helen: Dad, try to control yourself!

      C: (Helen and Victor walk out.)

      Helen: Victor, it seems that dollar was the first one dad ever made. It's a 1965 dollar and my dad signed and dated it on the back. We have to get it back for him!

      Victor: Sure!

      D: (In the storeswheresVictor bought the newspaper awhile ago)

      Cashier: I don't see a dollar like that in here. I must have given it to someone else.

      Helen: Oh, no!

      Victor: Who's been in here since me?

      Cashier: Let's see…old lady Sedgewick and Reggie!

      Victor: Helen, you take Mrs. Sedgewick. I'll take Reggie.

      Helen: Gotcha!

      (Mrs.Sedgewick answers the door.)

      Mrs.Sedgewick: A 1965 dollar, huh? Come on in, dearie, and I'll check. How about some tea first?

      (Victor runssintosReggie.)

      Victor: Reggie, you were just at the corner store7, weren't you?

      Reggie: What's it to you, Victor?

      Victor: I gave the guy a wrong dollar by mistake and he might have given it to you as change.

      Reggie: A special dollar, huh? Let's see if I have it and then we'll talk terms8!

      (Reggie takes several dollars out of his pocket.)

      Victor: It's not there! Were these the only ones you had?

      Reggie: Well, I did just pay back Richard a dollar I owed him!

      Victor: I've got to find Richard fast! A dollar won't last long in his hands!

      (At Mrs. Sedgewick's)

      Mrs.Sedgewick: Let's see!swheresdid I put my purse?

      Helen: Oh, brother!

      (Richard is walking on the street.)

      Victor: Richard! Hold it right there!

      Richard: Huh? What is it?

      (Victor grabs the dollar bill from Richard's hand and the bill is torn apart.)

      Victor: Mr. Dodge is really going to kill me now. I only have half of his lucky dollar!

      Richard: You mean this one with his name and a date on it?

      Victor: That's it! Richard, you're the greatest!

      (At Helen's home)

      Victor: Helen, I've got it!

      Helen: Thank goodness!

      Dad: At last! My lucky dollar is safe from Victor forever! Let me put it in this stamp album. I can relax now! Oh, gee! Helen! A rare 1806 uncancelled stamp must have fallen out of my album, did you find it?

      Helen: My goodness! Victor just left to mail a letter! You don't think…

      用法:The downfall of the company was caused by secret disclosure.公司的垮台是由于泄密。

      7 at the corner store:住宅区附近的商店

      8 terms: conditions with regard to payment付款条件,此句中talk terms为口语说法,意思是:谈谈条件看怎么回报我。

      1.scared me:吓我一跳

      用法:A noise from the tomb scared me.

      从坟墓里传出来的声音让我很害怕。

      2.be supposed to:应该,必须,被期望

      用法:Athletes who competed in the Olympic Games were supposed to be amateurs.

      在过去,奥运会的参赛运动员应该是业余选手。

      3.around the corner:在附近(不能理解成住在街的拐角处)

      用法:They only live just around the corner ---so we see them all the time.

      他们就住在附近,所以我们总能看到他们。

      4.live across town:live on the other side of the town本句指虽住在同一个镇上但彼此相隔的距离很远,相当于说:我住在镇的这头你住在镇的那头。在美国,town小于city大于village。

      5.none of your business:不干你的事。类似说法有:have nothing to do with you

      用法: They are teaching their children; it's none of your business.

      他们是在管教自己的孩子,又不关你的事。

      6.mess up:搞糟。在下文的"It's already a mess."中mess意为"一团糟"。

      用法(1) How could you mess things up so badly in one day?

      你怎能在一天之内把事情搞得如此一塌糊涂?

      (2)The house is a mess and I'm supposed to have friends to dinner tonight.

      这房子一团糟,而我今晚还要请朋友来吃饭。

      A construction site:建筑工地

      用法:On a construction site, everyone is required to wear a hard cap.

      在建筑工地,每一个人都要载安全帽。

      litter: n.(四处)乱扔垃圾,乱糟糟不整洁的状况。litter monitor意谓制止乱丢垃圾的监督员。

      用法:He is a litterbug. He never uses a litterbag to hold trash.他是个喜欢在公共场所乱丢垃圾的人,他从来不用小垃圾袋来装垃圾。

      B: (Ten minutes later, a cab arrives.)

      Claire: Bye, Mom and Dad!

      Mom: (Saying to Dad) You certainly relented5 in a hurry! You used to be so over- protective of your daughter.

      Dad: I still am, my dear. That is not an ordinary cab and driver I've called…I had this planned! My man, Alex is at the wheel6. He's a master of disguise7 and he'll follow Claire everywhere.

      Mom: Good grief!

      C: (In the cab)

      Claire: (Sitting in the back seat) Hmm! Cool shades! Imported Super-Egos! Uncommonly expensive. Rarely seen on cabbies!

      (Claire steps out of the cab and walks to the terminal8 building.)

      Alex: Take the cab back, Henry!

      Henry: Good luck, Alex!

      D: (In the coach of the plane)

      Claire: There's another pair of those imported Super-Egos on that redheaded9 man! What a coincidence! Yeah, coincidence?

      (Claire arrives at the beach.)

      Claire: Those disguises don't fool me! I'm sure dad's sent "Mr. Sunglasses" to watch over me!

      (Day Four)

      Claire: This is my last day and it calls for a donkey cart ride up to the famous Fort Ford that once guarded Oyster Bay. Maybe I can do this alone! Forget it! Hmm…I noticed our "guide" removed his Super-Egos while walking us up through Queen Vic Tunnel. I'm gonna show dad two can play this game!

    双月湾之旅~~意想不到的经历

    哈哈,上次闹了很久还是没有去成的双月湾,终于有机会去了,真的很开心。
    对于那个超陌生的地方,只是想看看有什么吸引的,去踏下超细的沙,捡捡贝壳,玩玩水,感受一下海水的味道,当然还有海边的日出与日落啦。。
    就这样两个人开着车冒黑,跟着GPS和手上的一张路线图出发了。。哈哈。。眨眼
    本来很顺利地经过三四个小时,晚上十点多终于摸索到达目的地架,但由于夜的关系,我地走左一段好冤枉的路,还要把车弄得遍体鳞伤,经历了在泥泞中走不到的惊险,好彩肥仔成够但定,我中惊部车就卡响果度,返吾到去了,林起都惊,点解我地甘勇架大笑(可惜 我惊到系识企响度,本来应该照照相,以作纪念架。。)
    嘻,今次中第一次训帐盘,比我想像的热多我,简直就系焗桑拿啊。。但好玩!
    不过跟住中经历左响车度训,中好玩,前座都可以平放的,我就好似训床甘,个个姿势都甘舒服,还有又软又暖的“抱枕”,哈哈,超爽的。。。
    期待着下一次的惊险。。
    原来生活真的需要激情才有意思,哈哈

    静静思。。

    最近让我心动,让我开心,让我烦恼,让我惧怕的事情特别多。。
    心动的是,原来被人“疼”的感觉真的是甜滋滋的;
    开心的是,原来“幸福”真的可以很简单;
    烦恼的是,怎么可以证实拥有的一切不是虚幻,家庭,爱情,事业,朋友都可以称心如意的程度又是多少呢?
    恐惧的是,越是美好的东西,越怕失去。。
     
    人,本来就是贪婪的动物,虽然知道很多事情在需要选择的时候,便会出现“有得有失”,往往在十字路口那里徬徨犹豫。。
    那是因为有些东西不想失去。。也害怕最后还是无可奈何地失去。。
     
    也许在这变幻莫测的世道,静下心来,想想自己最想要,最看重的是什么,然后=》能做的,想做的,就去做吧。。

    九种食物有效延缓衰老

     美国《时代周刊》近期刊登了一篇美国营养学家的文章,文中指出,身体内氧自由基不断增多是导致衰老的重要原因,在我们的日常食物中,有9种对抗氧自由基的能力特别强,可以起到延缓衰老的作用。 

      番茄: 

      很多研究都表明,烹制过的番茄可以降低人类患前列腺癌和其他癌症的危险。这都要归功于番茄里含有的番茄红素,它是同类营养素中最强有力的抗氧化剂。除此之外,其中的茁胡萝卜素在消灭氧自由基方面也很出色。 

      因此,别看西红柿小,其实它给我们带来的益处还大着呢!据研究测定:每人每天食用50克-100克鲜西红柿,即可满足人体对几种维生素和矿物质的需要。 

      菠菜: 

      菠菜里富含铁和叶酸。叶酸不但可以预防小儿神经管缺失,还可以降低高半胱氨酸(一种可以刺激血管和导致心脏病的氨基酸)在血液中的浓度。此外,菠菜中含有叶黄素和玉米黄质,它们可以阻止失明的头号“杀手”——视网膜黄斑性病变。 

      坚果: 

      坚果像一个小型的营养发动机,其中含有对人体有益的脂肪酸和维生素E。 

      坚果是植物的精华部分,一般都营养丰富,含蛋白质、油脂、矿物质、维生素较高,对人体生长发育、增强体质、预防疾病有极好的功效。 

      燕麦: 

      每天吃点燕麦可以起到降低胆固醇的作用,因为它含有茁葡聚糖,是一种海绵状可溶解的纤维素,可以让早期生成的胆固醇化于无形。 

      花椰菜: 

      其中含有很多植物化学因子,可以在癌细胞形成之前将它们消灭掉。它还含有胡萝卜素、纤维素和维生素C。最好的吸收方法就是稍稍炒一下,吃时多咀嚼。类似的蔬菜还有甘蓝和卷心菜。 

      鲑鱼: 

      这种鱼主要以吃海藻为生,而海藻可形成一种特别的脂肪,就是我们熟知的欧咪伽—3脂肪酸,对于心脏非常有益,还可以抑制与自身免疫系统相关的疾病。 

      大蒜: 

      大蒜里含有的植物化学因子对心脏有益。为了让它发挥最大的功效,最好把大蒜切碎或者捣碎食用,吃时不要长时间加热,否则会丧失它的有益成分。 

      绿茶: 

      绿茶内含有的多酚是一种健康作用百倍于维生素C的植物化学因子。日本研究表明,每天饮用10杯绿茶可以减低患心脏病的危险,用它做漱口水还可以抑制口腔细菌的生长。 

      蔓越橘: 

      蔓越橘中所含的抗氧化剂比任何水果和蔬菜都多,其中的花青素对心脏病和癌症的治疗很有帮助,还可以防止大肠杆菌附着在膀胱壁上,导致尿道炎。
